Parent / Guardian Consent Form

This form outlines the permissions required for a student to participate in Collabs. By agreeing, parents/guardians confirm their understanding of the program's benefits, risks, and policies within our independent digital community.

1Participation & Safety

My child will participate voluntarily in Collabs, a secure digital platform for e-learning and co-curricular activities.
My child must complete KYS (Know Your Student) verification for safety, and will be assigned a unique Collaber ID.
No personal information (photo, contact details) will be shared publicly.
All interactions are moderated, and my child must follow the Code of Conduct, including strict rules against bullying.

2Data & Privacy

All data is protected under the Privacy Policy and will be permanently deleted upon leaving the community.
I consent to my child's data being used for internal verification and program improvement only.
No data will ever be shared with third parties without my explicit consent.

3Liabilities & Withdrawal

I understand there are absolutely no financial obligations or liabilities associated with this program.
Collabs is not liable for external platform changes, device problems, or technical issues.
Participation can be withdrawn at any time by notifying the Executive Team in writing, at which point all access ends and data is deleted.
